で「日本で、先週日曜までの週の一医療機関ごとのインフルエンザ患者数が、調査が開始された1999年以来、過去2番目に高いレベルに達した。厚労省が発表した。」という意味になります。

The number of influenza patients per medical institution in Japan in the week through last Sunday までが主語（主部）になります。
「インフルエンザ患者数が、医療機関ごとの、日本で、先週日曜までの週の中での」です。
「先週日曜までの週中の日本の一医療機関毎のインフルエンザ患者数」となります。
influenza patient で「インフルエンザ患者」です。
influenza は略して flu フルーとも言います。
例：flu epidemic「インフルエンザの大流行」
per medical institution で「一医療機関あたりの」です。
医療機関ごとの平均値ということです。
per ～ は「～につき、～毎の、～当たりの」という意味です。
per の後には a(an) が付かないので注意しましょう。
例：per second「一秒あたり」、per week「一週間につき」
in the week through last Sunday で「前回の日曜までのその週内で」です。
この場合の through は「～が（完了する）までの」という意味合いになります。
例：through the end of Ramadan「ラマダンが終わるまで」、numbers one through ten「1から10までの数」

hit the second-highest level で「過去二番目に高いレベルを記録した」です。
hit a ～ level で「～のレベルに達する」です。
この場合の hit は過去形です。
hit は過去・過去分詞形とも同じ形になる動詞です。

since the survey started in 1999 で「1999年に調査が開始されて以来」です。
survey [サ]ーヴェイは「調査、測量、世論調査」という意味の名詞（noun）です。
動詞として使う場合はサー[ヴェ]イと後ろにアクセントが来て「～を（詳細に）調査する、測量すう」という意味です。

the health ministry has said. で「厚労省が発表した」です。
the health ministry は「保健省」ですが、この場合は日本ですので「厚生労働省」を指しています。
厚生労働省は正式には Ministry of Health, Labour and Welfare（MHLW）です。
