America’s president has signed an executive order signalling that his country would withdraw from the UN agency. で「米大統領は、同国がこの国連機関から脱退することを示唆する大統領令に署名した。」という意味になります。
Donald Trump has once again set his sights on ～ で「ドナルド・トランプ氏が再び～に狙いを定めた」となります。
once again は副詞で「またまた、再び」等の意味になります。
set one’s sights on ～ で「～に照準を合わせる」です。
sights は「（猟銃などの）照準」です。複数形となります。
他にも aim one’s sights on ～ などとも言えます。
(on) the World Health Organisation (WHO). で「世界保健機関（WHO）に」です。
America’s president has signed an executive order で「米大統領は、大統領令に署名した」です。
America’s president はドナルド・トランプ氏のこと。
sign an executive order で「大統領令にサインする、署名する」です。
executive order はアメリカでは「大統領行政命令」という意味になります。

その大統領令は、
signalling that ～ で「～ということを示唆している」です。
signal ～ で「～を示唆する、～の前兆となる」
何を示しているのかというと、
(that) his country would withdraw from the UN agency. で「彼の国がその国連機関から脱退するだろう（ということ）」です。
his country とはアメリカのこと。
withdraw from ～ で「～を脱退する、手を引く、撤退する」です。
the UN agency は the World Health Organisation を言い替えて「その国連機関」です。
UN は U.N. とも書いて、United Nations の略。

それではまず文節に分けて日本語に訳し内容を理解して下さい。
