The cornerstone is ～ で「その要諦は～」です。
cornerstone は建築で土台に使う「隅石」からきて「基礎、肝要な部分」という意味で使われます。
his proposal to ～「～するという彼の提案」です。
(to) send money directly to Americans「お金を直接アメリカ国民に給付する（という提案）」です。
for health savings accounts で「医療貯蓄口座（HSA）のため（のお金を）」です。
health savings account（HSA）は、アメリカでよく使われる「医療費専用の節税口座」です。
医療費を貯める口座ですが、非常に有利な節税効果が得られます。
医療保険制度に、投資・節税口座（NISA的な）を合体させたような制度ですね。
so ～「～するように」
they can handle insurance and health costs で「国民が保険や医療費を管理できる（ように）」
as they see fit.「自分たちの好きなように」です。
handle ～ で「～を処理する、管理する」
as ～ see fit で「～が好きなように、～がいいと思うように」です。
ここでの fit は形容詞で「適合した、適切な、ふさわしい」です。

reject ～ で「～を拒絶する、却下する、受け入れない」
(as) a paltry substitute for ～ で「～のお粗末すぎる代替でしかない（として）」です。

paltry で「ほんのわずかな、取るに足りない、お粗末な、しょぼい、みみっちい」です。
(for) the tax credits「税額控除（のかわり）」です。
tax credit で「税額控除」です。
that had helped ～ で「～するのに役立ってきていた（税額控除）」です。
(helped) lower monthly premiums で「月額保険掛金を下げる（のに役立ってきた）」です。
lower で「～を下げる、低くする」という意味の動詞。
