Population aging で「人口の高齢化」という意味になります。
population は「人口」、aging は「高齢化、老化」という意味の名詞です。
例：moderate population aging「人口の高齢化を抑制する」

an exodus of young people to the cities は「若い人々が都会に出て行ってしまうこと」です。
exodus はもともとは旧約聖書の「出エジプト」のことで、「集団で人口が出て行くこと、人口の流出」を意味します。
例：holiday exodus「出国ラッシュ」、rural exodus「農民離村」

have made it hard for many of these centuries-old traditions to be maintained で
「これら数世紀続いてきた多くの伝統を維持していくことを難しくしている」という意味になります。
make it hard for ～ to … で「～が…することを難しくする」という意味になります。
make は「～を…の状態にする」という意味ですので、hard の部分はいろいろ置き換えて使えます。
例：make it possible for ～ to …「～が…できるようにする」、make it easier for ～ to …「～が…しやすくする」
centuries-old traditions で「数世紀続いた伝統」です。
centuries-old は「数世紀続いた、何世紀も続く、数百年にわたる」という意味の言い回しです。
