For 68 weeks, で「68週の間」です。
月4週間とすると17カ月ですね。

participants injected themselves weekly with ～ で「参加者たちは、自分に注射した、毎週、～を」です。
participant は「参加者」という意味です。
clinical trial study「治験」に参加した人たちのことですね。
inject ～ with … で「～に…を注射する」という意味になります。

何を注射したかというと、
with either semaglutide or a placebo. で「セマグルチドかプラシーボのどちらかを」です。
either ～ or … の形で「～か…のどちらか」となります。
semaglutide は日本では「セマグルチド」と呼ばれているようですが、
英語での発音は、セマグルタイドゥです。
placebo は「偽薬、プラシーボ、プラセボ」です。
薬として効く成分は入っていません。よく臨床試験で新薬や治療法の効果を検討するために使われます。

On average, 「平均で」

over half of those who received the drug itself までが主語です。
「～した人たちの半数以上、その本物の方を投与された（人たち）」です。
those who ～ は「～する人々」という意味の言い回しです。
どんな人たちかというと、
(who) received the drug itself で「その薬の本物の方を受けた（人）」です。
receive a drug で「薬を投与される」という意味のコロケーションです。
この itself は、either semaglutide or a placebo を受けて、
偽薬ではない方、つまり「本物の・実物のその薬」という意味です。

その人々の半数が、
lost nearly 15% of their body weight, で「15%近く体重を減らした」です。
lose one’s weight で「体重を減らす、減量する」という意味のコロケーションです。
lost は lose の過去形ですね。

and「そして」
over a third が主語で「3分の1以上の人たち」です。
achieved a weight loss で「減量を達成した」です。
weight loss で「減量」という意味になります。

(a weight loss) of at least 20% of their weight. で「少なくとも彼らの体重の20%という（減量）」です。
