で「今日、いつものことながら、私がとても胸が小さく子供のような見た目なので、彼はレストランで節約するため、わたしを妹ということにしてうまいこと押し通した。」という意味になります。

as always は「いつもどおり、いつものように」という意味の言い回しです。

so ～ that … の形ですね。「あまりに～なので…だ」という意味になります。

flat-chested は「（女性が）胸が小さい、ペチャパイの、平らな胸の、貧乳な」という意味です。
逆の意味で「巨乳の」はなんていうかというと、large-breasted と言います。
他にも big tits もよく使われます。

childlike in appearance で「見た目が子供みたいな」です。
in appearance は「見た目上、みたところ、」という意味になります。
例：similar in appearance「見た目が似ている」

that 以降は、so flat-chested and childlike in appearance 「とても貧乳で子供みたいな見た目なので」どうなのかという「結果」が続きます。

pass me off as ～ で「私を～として通す、成りすませる」という意味になります。
pass off は「うまくやり過ごす」という意味の句動詞です。
例：pass off imported beef as domestic「輸入牛肉を国産と偽って売る」

little sister は「妹」です。「姉」は big sister です。
他にも、younger sister と elder sister という表現もあります。

それではまず文節に分けて日本語に訳し内容を理解して下さい。
