Today, while sitting down on the dentist’s chair, the tooth to be removed came out on its own. I still had to pay $50 for the cotton and advice. FML
で「今日、歯医者の椅子に腰かけていたら、抜くはずだった歯がひとりでに抜けた。私はそれでも歯に詰める綿とアドバイスに$50ドル払わなければらならなかった。F*ck my life!」という意味になります。

while sitting down on the dentist’s chair で「歯医者の椅子に座っていた時、座っていたら」という意味の言い回しです。
while は「～の間に、～中に」という意味の接続詞です。

the tooth to be removed came out on its own で「抜くはずだった歯がひとりでに抜けた」という意味になります。
the tooth to be removed までが主語です。
to be removed で「抜くはずの、抜く予定の」という意味になります。
to be ～ は「～する予定の」という意味があります。
例：the meeting to be held on「開催予定の会議」、husband-to-be「夫になる予定の人」
come out は「出てくる」という意味ですが、歯に使う場合は「（歯が）抜ける」という意味になります。
例：My tooth came out.「歯が抜けた。」
on its own は「ひとりでに、勝手に」という意味の言い回しです。
例：heal on its own「自然に治る」、a door slams shut on its own「ドアが勝手にバタンと閉まる」

I still had to pay ～ で「私はそれなのに払わなければならなかった」です。
still は「それなのに、それでも、にもかかわらず」という意味があります。

それではまず文節に分けて日本語に訳し内容を理解して下さい。
