Halima Ali Maiyanga told her family in a phone call Thursday (that) ～ で「ハリマ・アリ・マイヤンガさんは、家族に伝えた、電話で、木曜日に、～と」です。
told は tell「～に伝える」の過去形です。
tell ～ … で「～に…を伝える」となります。ここでは … には that節以降の内容が入ります。
in a phone call で「電話で、電話口で」です。

なんと伝えたかというと、
(that) she was one of hundreds of captives who ～ で「自分が～の中の一人だったと、数百人の～な人質の」です。
one of hundreds of captives で「数百人の人質の中の一人」という言い回しです。
hundreds と複数形で「数百」という意味になります。
captive は「人質、捕虜、捕らわれた人」という意味の名詞（noun）です。
例：make someone captive「（人を）捕虜にする」
(captives who) were able to flee で「逃げることができた（人質たち）」です。
flee は「逃げる、逃れる」という意味の動詞（verb）です。
悪いことをして逃げるというよりも、「（危険を回避して、安全な場所に）逃げる、非難する」という意味合いです。
例：flee abroad「国外に逃げる」

amid ～ で「～のさなかに、～の渦中で」
(amid) a Nigerian military offensive against Boko Haram fighters で「ナイジェリア軍の軍事攻撃（のさなかに）、ボコ・ハラムの兵士たちに対する」です。
a Nigerian military offensive で「ナイジェリア軍の軍事攻勢」となります。
offensive は「イライラさせるような、侮辱的な、けんか腰の」という意味の形容詞でもよくつかわれますが、
ここでは名詞で「（軍事）攻撃、攻勢」という意味です。
例：begin an offensive「攻撃を開始する」、cast an offensive eye at ～「～に攻撃的な目を向ける」

in the Sambisa Forest in the country’s northeast. で「サンビサ・フォレストの中で、同国の北東部の」です。
the Sambisa Forest は地名です。
