Today, it’s the fourth day since I got home from hospital in a wheelchair after a car wreck. It was also the first day that someone in my house offered to do something for me. My father-in-law visited and offered to make me a sandwich. My response? I cried. My wife and kids laughed at me. FML

Today, it’s the fourth day since I got home from hospital in a wheelchair after a car wreck. It was also the first day that someone in my house offered to do something for me. My father-in-law visited and offered to make me a sandwich. My response? I cried. My wife and kids laughed at me. FML
で「今日、自動車事故の後、車椅子に乗って帰宅してから四日目だ。そして家の誰かが私に何かをしてあげると言ってくれた初めての日だった。義理の父がやってきてサンドイッチを作ると言ってくれた。私の反応は？ 泣いた。妻と子供たちは笑っていた。F*ck my life!」という意味になります。

it’s the fourth day since ～ で「～して以来、（今日は）4日目だ」です。
ここでの it is ～ は日付・時刻・天気などを表すときの言い回しですね。
the fourth day で「四番目の日（四日目）」です。

since ～「～して以来」
I got home from hospital で「病院から帰宅して（から）」です。
get home from ～ で「～から帰宅する」という意味の言い回しです。
in a wheelchair で「車椅子に乗って」という意味になります。
日本人感覚ですと on a wheelchair と言いそうですが、車椅子は in ～ で表現します。
背もたれや肘掛つきの椅子やソファーなどの場合は、このように in ～ で表すことが多いです。
例：She sat in the chair.「彼女は椅子に座った。」

after a car wreck で「自動車事故の後で」です。
car wreck で「自動車事故、交通事故」です。
car crash, car accident などとも言い換えられます。
wreck [ゥレ]ック はもともとは難破船・沈没船という意味合いの名詞です。
そこから、難破する、倒壊する、損壊する、ボロボロになる、大破などという意味合いになります。
例：wreck someone’s dream「夢をぶち壊す」、underwater plane wreck「沈没した飛行機の残骸」

It was also the first day (that) ～ で「また同時に、（～な）初めての日だった」です。
the first day で「初めての日、第一日目」です。
(that) someone in my house offered to do something for me. で「我が家の誰かが、私のために何かをしてくれると言った（初めての日）です。」
someone in my house で「我が家の誰か」です。
offer to do something で「何かをしてあげると申し出る」です。
offer は「～をあげると言う、提供を申し出る」という意味の動詞です。

My father-in-law visited and offered to make me a sandwich. で「義理の父が訪れて、私にサンドイッチを作ってやると申し出た」です。
father-in-law で「義理の父」です。
～-in-law の形で「義理の～、血のつながりのない～」という意味になります。
結婚によって家族関係となった相手を表します。
例：brother-in-law「義兄弟」、son-in-law「義理の息子、娘婿」

My response? I cried. で「私の反応は？ 泣いた。」です。
My wife and kids laughed at me. で「妻と子供たちは私を笑っていた」です。

それではまず文節に分けて日本語に訳し内容を理解して下さい。
