フロリダで14歳の少年が13歳の少女を殺害したというニュースです。

He offered new grisly details about ~ で「彼は新たな~についてのおぞましい詳細を語った」となります。
記事を読むと、He は、State Attorney R. J. Larizza「州検事のR.J.ラリザ氏」のことです。
プレス・カンファレンスで事件の詳細を語った場面です。
offer は「~を提供する、提示する、あげると言う」ですが、ここでは「(見解などを)述べる」という意味で使われています。
例:offer no personal opinions「個人的な意見を一切語らない」、offer one’s congratulations「祝辞を述べる」
grisly グリズリィは「ぞっとするような、おぞましい」という意味の形容詞です。
例:grisly crime「忌まわしい犯罪」、grisly scene「ぞっとするような光景」
ちなみにグリズリー・ベアーのグリズリーは
grizzly で意味は「灰色の」、発音は同じの同音異義語(homophone)です。

noting (that) ~ で「~だということに言及して、~に触れて」という意味になります。
これ以降は、分詞構文「~しつつ、~しながら」という意味の従属節となります。

there were 114 stab wounds found on Bailey, で「114の刺し傷がベイリーの体に見つかった」です。
there were 114 stab wounds で「114の刺し傷があった」
found on Bailey で「ベイリー(の体)に発見された(刺し傷)」です。
found find の過去分詞形です。
Bailey は殺害された少女の名前です。

citing the medical examiner report, でさらに分詞構文の挿入句が入り「検視官の報告を引用しつつ」です。
cite は「~を引用する、~に言及する」です。
medical examiner は「検視官、検察医」です。

and (that) ~ で「そして(~と言うことにも言及して)」
at least 49 of those wounds で「少なくともそれらの傷のうち49個は~」
those wounds「それらの傷」とは前述の114個の傷のことですね。
そのうちの49個の傷は、
were to the hands, arms and head「両手両腕、頭部へ(の傷)だった」です。
hands は「手」で、手首から先の部分、arms は手首から肩までの部分です。
head は顔も含んだ「頭部」です。

(that) were defensive in nature. で「(それらの傷は)性質上、防衛的なものだった」です。
要するに、手や腕、頭部への傷は、ナイフでの攻撃を防ごうとしてできた傷だということですね。
defensive で「防衛的な、防御的な」という意味の形容詞です。
in nature は「本質的に、性質上、性格的に」という意味の副詞句です。
例:He is aggressive in nature.「彼は攻撃的な性格だ。」

それではまず文節に分けて日本語に訳し内容を理解して下さい。
