The gunman opened fire at three locations, で「その銃を持った男は発砲した、３か所で」です。
gunman で「拳銃使い、殺し屋、武装した犯人」という意味になります。
open fire で「発砲する」という意味の句動詞です。

including ～「～を含めて、～なども」です。
その三か所の中には～も含まれる、ということです。
outside the London Pub, で「ロンドンパプの外」です。
London Pub はキャピタライズされているので固有名詞だと分かります。
a popular gay bar in downtown Oslo. は the London Pub と同格的に言い替えた名詞句で「人気のゲイバー、オスロのダウンタウンにある」です。
downtown ～ で「～のダウンタウン」という意味の言い回しです。
例：downtown Tokyo「東京の繁華街」
downtown は「下町」ではなく、都市の中で一番賑やかな繁華街、商業地、中心街という意味です。

Police investigators have said (that) ～ で「警察の捜査官たちは～だと言っている」です。

it is too early to say ～ で「～を発言するにはまだ早すぎる、早計である」です。
it is too early to do ～ で「～するにはまだ早すぎる」という意味の言い回しになります。
何を発言するにはまだ早すぎるのかというと、
whether the attacker specifically targeted the LGBTQ community. で「その襲撃者が、特にLGBTQコミュニティーを攻撃対象にしたかどうかを」です。
whether ～ は「～かどうか」という意味の接続詞です。
the attacker は先ほどの the gunman の言い換えですね。
specifically スペ・[スィ]・フィ・カ・リで「特に、具体的に」という意味の副詞です。
LGBTQ は Lesbian, Gay, Bisexual, Transgender, Queer の略で「性的マイノリティー」という意味合い。
