で「メディケアやメディケイドや社会保障制度などを通して私たちが互いに誓い合うことは、我々アメリカ人の先取性をダメにするものではなく、我々を強くしてくれるものです。それらは我が国を受け取るだけの者たちの国にするものではなく、この国を偉大にするためのリスクがとれるよう我々を解放してくれるものなのです。」という意味になります。

The commitments (that) we make to each other で「私たちがお互いに誓い合う約束事」です。
make a commitment to ～ で「～に誓いを立てる、～に確約する」という意味の言い回しです。
through Medicare and Medicaid and Social Security, で「メディケアやメディケイドや社会保障制度などを通して」です。
いわゆるオバマケアと呼ばれる「医療保険制度改革」のことですね。

these things do not sap our initiative, で「これらのことは、～するものではない、私たちの先取的な国民性を駄目にするものではない」です。
these things は The commitments のことを同格に言い替えています。
sap は「～の活力を徐々に奪う」という意味の動詞です。
名詞では「樹液、活力」という意味なのでそこから「樹液を搾り取る」という意味になっています。
our initiative で「私たちの先取性」です。
initiative は「自発性、先取性」と言った意味の名詞です。
他にも「新しい取り組み、先駆けた取り組み」という意味でもよく使われますね。
(but) they strengthen us. で「（そうではなくて）それらは私たちを強くしてくれる」です。

They do not make us a nation of takers で「それらは、わが国を～にするものではない、ただ貰いの国に」です。
make ～ … で「～を…にする」です。
us は「私たち（アメリカ）」を指して、それを何にするのかというと、
a nation of takers で「受けとる者たちの国」です。
つまり give and take の give はせずに take だけする人 takers です。

they free us to take the risks で「（そうではなく）それらは私たちを～するために自由にする」です。
free ～ で「～を自由にする、～を解放する」です。
何をするために開放するかというと、
to take the risks で「リスクを取るために、リスクが取れるように」です。
どんなリスクかというと、risks を主語に取った that で続けて、
(that) make this country great. で「この国を偉大にする（リスク）」です。
この場合の risk は「危険」という意味よりも「挑戦のために必要なリスク」という意味合いですね。

それではまず文節に分けて日本語に訳し内容を理解して下さい。
