主語は、

fell for the 37th consecutive year で「37年連続で落ちた」となります。
fell は fall フォール「（数量などが）落ちる、下がる、減少する」の過去形です。
the 37th consecutive year の形は、先日も出てきましたが、「連続37年」という意味になります。
the __th（序数）の形の時は、year が複数形にならないことにも注意しましょう。
例：for the 5th consecutive day「3日連続」、for two consecutive nights「二夜連続で」

to a new record low で「過去最低となる」です。
record low で「最低記録」という意味の言い回しです。
反対は record high で「過去最高」です。
例：hit a record low「過去最低となる」

ニュース記事でよくある言い回しです。

長いので、ちょっと切り分けが難しいですが、
indicate (that) ～「～ということを示している」の形です。
that 節の主語は、

「安倍政権による長期低出生率問題に対抗する努力」です。
efforts by Prime Minister Shinzo Abe’s administration で「総理大臣安倍晋三の政権による努力」です。
Prime Minister は「総理大臣」です。アクロニム（頭字語）は PM となります。
to combat the long-running low birthrate challenge で「長引く低出生率問題に対抗するための」です。
combat は「敵と闘う、戦闘する」という意味ですが、
この場合のように「問題に取り組む、対抗する」という意味でもよく使われます。
low birthrate challenge で「低出生率問題」です。
