Today, I told my grandmother I was planning a trip to take my newborn son to meet his great-great-grandfather before it was too late. She had a guilty expression on her face. My great-grandfather died 4 months ago and my family forgot to tell me. FML
で「今日、生まれたての息子を、手遅れになる前に（息子から見て）ひいひいおじいちゃんに会わせに連れて行こうと思っていると、おばあちゃんに伝えたら、おばあちゃんは後ろめたそうな顔つきになった。私のひいおじちゃんは4ヶ月前に亡くなっていて、私の家族は私にそれを知らせるのを忘れていた。F*ck my life!」という意味になります。

I told my grandmother (that) ～ で「おばあちゃんに～と伝えた（話した）」です。

なんと話したかというと、
I was planning a trip to ～ で「私は～する旅を計画している」です。
trip は「旅」ですが、「ちょっとした遠出」という意味でも使われます。

どんな旅かというと、
to take my newborn son to meet his great-great-grandfather で「彼にとってはヒイヒイおじいちゃんに会わせるために生まれたての息子を連れて行く」です。
newborn は「生まれたての」という意味の形容詞です。
例：newborn baby「生まれたての赤ちゃん、新生児」
great-great-grandfather で「ひいひいおじいちゃん」となります。
before it was too late で「遅過ぎになる前に、手遅れになる前に」です。
ヒイヒイおじいちゃんが亡くなってしまう前にという意味ですね。

She had a guilty expression on her face. で「おばあちゃんは後ろめたそうな表情を浮かべた」です。
guilty expression で「後ろめたそうな表情」となります。
guilty は「有罪の」という意味ですが、この場合は「罪の意識のある、後ろめたい」という意味になります。
例：guilty feeling「罪悪感、後ろめたさ」

My great-grandfather died 4 months ago and my family forgot to tell me. で「私のひいおじいちゃんは4ヶ月前に亡くなっていて、私の家族はそれを私に知らせるのを忘れていた」となります。
great-grandfather で「ひいおじいちゃん」となります。
息子にとっては、ヒイヒイおじいちゃん（his great-great-grandfather）ですが、
お母さんにとっては、ひいおじいちゃん（my great-grandfather）ですね。

それではまず文節に分けて日本語に訳し内容を理解して下さい。
