で「大衆文化の中では手洗いが最も一般的なOCDのイメージですが、強迫観念や衝動行動は、様々な形を取ることがあります。強迫観念は、汚染や病気への恐怖、人を傷つけてしまうという不安、数やパターン、道徳や性同一性などへの執着などとして現れることがあります。」という意味になります。

Although ～「～だけれど、～ではあるが」
hand washing is the most common image of OCD in popular culture, で「大衆文化の中では、手洗いが、最も一般的なOCDのイメージである（けれども）」
popular culture で「大衆文化、庶民の文化、ポップカルチャー」です。
popular には「人気の」という意味以外にも「大衆の、庶民的な、一般向けの」という意味のがあります。
obsessions and compulsions can take many different forms. で「強迫観念や衝動行動は、様々な形を取りうる。」です。
obsessions と compulsions は何度も出てきていますが、
obsession が「頭に取りついて離れない考え、強迫観念、妄想」で、
compulsion が「（理性では抑えられない）衝動、衝動的行動」です。
take a ～ form で「～な形態をとる」という意味のコロケーション。
例：take a very unusual form「極めて変則的な形をとる」

Obsessions can manifest as ～ で「妄想は～として現れることがある」です。
manifest は「はっきりした、明白な」という意味の形容詞でおなじみですが、ここでは動詞で「（兆候などが）現れる」という意味です。
fears of contamination and illness, で「汚染や病気への恐怖」
worries about harming others, で「他者を傷つけることについての心配」
or preoccupations with numbers, patterns, morality, or sexual identity. で「数、パターン、道徳、性同一性への執着」です。
列挙のカンマが入り乱れてちょっと分かりづらいですが、
上記三つ、fears of ～, worries about ～, or preoccupation with ～ が、「～, ～, or ～」の形で列挙され、
さらに、最後の preoccupation with ～ の句の中で、
numbers, patterns, morality, or sexual identity が同じく、「～, ～, or ～」の形で列挙されています。
preoccupation プリアキュ[ペ]イションは「没頭、先入観、執着」といった意味の名詞です。
形容詞形は preoccupied プリ[オ]キュパイドゥ「頭がいっぱいで、～に夢中になって」という意味です。
occupy オキュパイは「～を占領する、占有する」という意味ですので、頭の中を「先に占領された」状態ですね。

それではまず文節に分けて日本語に訳し内容を理解して下さい。
