I’m selfish, impatient, and a little insecure. I make mistakes, I’m out of control, and at times hard to handle. But if you can’t handle me at my worst, then you sure as hell don’t deserve me at my best.
で「私は我がままで我慢ができない性質でちょっと精神的に不安定なときもある。間違いもするし言うことも聞かない。そして時には扱いきれない女よ。でも私の最悪な状態に上手く付き合えないのなら、貴方は私の最高の状態には絶対にありつけないわ。」という意味になります。

人の欠点を表す表現がたくさん出てきました。
selfish は「わがままな、自己中心的な」と言う意味の形容詞です。
impatient で「我慢ができない、せっかちな、すぐにイライラする」です。
patient「我慢強い」に、否定の接頭語 im- が付いた形です。
insecure は「不安な、自信がない、精神的に不安定な」です。
例：insecure feeling「心もとない感じ、心細さ」

out of control は「制御できない、どうにもできない、自制心のない」と言う意味のイディオムです。
hard to ～ で「～し難い、～することが難しい」です。
hard to handle なので「扱うことが難しい、対処し難い」です。

if you can’t handle me at my worst で「わたしが最悪の時に私とうまく付き合えないなら」です。
at my worst で「最悪の時」、at my best で「最高の時」です。

you sure as hell don’t deserve me at my best で「貴方は間違いなく最高の時の私と付き合う資格はない」となります。
sure as hell は話し言葉で「間違いなく、絶対に」と言う強調の意味になります。
deserve は「～にふさわしい、～を受けるに値する」と言う意味の動詞です。
例：deserve a better man「もっといい男と付き合う価値がある」、deserve a reward「報酬を受けるに値する」

それではまず文節に分けて日本語に訳し内容を理解して下さい。
