「かつてキリスト教徒が残忍に迫害されたという日本の南の地域にある12箇所で構成される潜伏キリシタン遺跡」です。
A dozen Christian locations で「12のキリスト教徒の場所」となります。
dozen は「１ダース、12個の」という意味です。
in parts of southern Japan で「南日本のいくつかの地域にある」です。
日本語では「南日本」という呼び方はあまり聞きませんが、英語ではしばしばこういう言い方がされますね。
今回の遺跡は、長崎県、熊本県に点在しているようです。
where members of the faith were once brutally persecuted は A dozen Christian locations を修飾する関係代名詞節です。
「（そこでは）その宗徒たちがかつて無残に迫害された（場所）」となります。
members of the faith で「その宗教の宗徒たち、信仰の徒」となります。
faith は「信頼、信念、信仰心」という意味から「宗教」という意味でも使われます。
were once brutally persecuted で「かつて残忍に迫害された」となります。
once は「かつて、以前は」という意味です。
